Kirkpatrick St. SERVICES: 24/7 Helpline/Information & Referral - Trained staff and volunteers are available 24/7 to answer questions about Alzheimer’s disease and dementia, provide emotional support, and give referrals to community resources. Care Consultation - Staff is available to assist persons with dementia and their families solve immediate problems and plan for the future. Services provided in-person, by telephone, or by e-mail. Support Groups - There is one support group that meets the first Tuesday of each month at 1:30pm at Mercy Health & Rehabilitation Center, 3 St. Anthony St., Auburn. For more information about the support group, please call (800) 272-3900. Family Caregiver Education - Education programs are provided to family caregivers on such topics as basics of Alzheimer’s disease and dementia, communication strategies, behavior management, coping skills and ways to maintain brain health. Professional Education/Training - Professional and support staff of health care agencies, community residences, nursing homes and other community organizations can receive in-service training regarding Alzheimer’s disease and related dementias. Medic Alert & Safe Return- A nationwide system designed to locate, identify, and safely return individuals with dementia who wander and become lost. Identification products, including a bracelet or necklace and clothing labels are provided, and the individual is enrolled in the national database which can be accessed by a 24-hour, toll free number. For more information, contact Chapter Office at (800) 272-3900. Kirkpatrick Adult Day Program – A dementia specific social adult day program in Syracuse. Open Monday-Friday, 7:30am to 5:30pm.
